Dr Keenan was born in Ontario, Canada and lived in many places in the province and country. As an undergraduate, he was balancing passions for medicine and wildlife ecology until medicine won out. He received his medical degree from the University of Alberta then moved to Toronto to complete a residency in general surgery and a fellowship in thoracic surgery. In between, Dr Keenan spent two years at the University of Pittsburgh where he conducted research on lung transplantation. He returned to Pittsburgh, in 1992, for a faculty position after completing his thoracic surgical fellowship.
