Care team includes a RN, a medical social worker, a hospice aid, a spiritual care counselor and a volunteer. Also involved is our medical director and the patient's primary care doctor.

Services
Hospice helps people facing end-of-life decisions to choose how their final days will be spent, free of pain in a safe and compassionate environment. Hospice helps people facing end-of-life decisions to make choices. Choices that will allow them to spend their final days with dignity and comfort, surrounded by their loved ones. The goal of hospice is to enable patients to continue an alert, pain-free life while managing other symptoms that may occur.
Palliative Care is s offered to those who have been diagnosed with a life-limiting illness, but are still seeking curative treatment. Using a combination of active and compassionate therapies the program’s goal is to prevent or treat the symptoms of the illness and the side effects caused by treatment challenges. This includes addressing psychological, social and spiritual challenges related to the illness or treatment.
Our comprehensive counseling and grief support programs are offered to family members, friends and loved ones, providing the support needed to adjust and resume their lives after the loss of a loved one. Our services are free to family members, friends and loved ones of patients on our services. For community members outside of our service seeking individual or family grief counseling, we offer a sliding fee scale based on yearly household income. The first session is always free.
